Medicaid Fee Schedules


Instructions for importing disclosure files: The files below with an EXE extension are self-extracting ZIP files. To download them, click on them with your mouse. You will be asked to store them somewhere. Save them to a temporary directory like c:\temp\. Once they are downloaded, Execute them to EXTRACT the appropriate text files out of them. You may them delete the EXE file. These files are saved in tab-delimited, ASCII text format (with a .TXT file extension). You should use either a spreadsheet or database to access these files, as the data is stored in column format. Most major programs of these types will directly support the import of tab-delimited text - see your software manual for specific instructions.

The data is laid out in the following format:

Once imported, the data will most likely need to be formatted for better readability. You should adjust column widths to display all text (often, there is an option to automatically make these adjustments to the full width of the data). Also, some procedure codes may look shortened - leading zeros will not appear unless you format numerical data to display up to five digits.

You can also use "search" commands in many programs to jump you directly to specific procedure codes, types of service, etc.

The report can also be formatted by your specific software and printed if your office prefers the paper report media.

Note to ASC providers (provider types 51 and 52): Since your pricing is based on locality, you need to be sure you choose the correct disclosure file. Each ASC disclosure report is named as follows: ASC$$$.txt (where $$$ = 3-digit locality code).
